No. You pay your plan price plus $0.50 for each appointment, with no minimum and no setup fee.
Each appointment counts once, in the month it starts. An appointment that is later canceled still counts.
Every agency plan is free for 30 days, with no charge for appointments during the trial. You add a card at signup and are not charged until the trial ends.
Yes, in Settings › Subscription. An upgrade starts right away and you pay the prorated difference. A downgrade starts at the end of your billing period.
